Job description

Keystone is a not-for-profit 501(c)(3) organization in York County, South Carolina. We offer internationally accredited and licensed services including education, prevention and treatment to meet the needs of individuals, families, and groups that are experiencing alcohol and/or drug related problems. We are seeking a full-time Case Manager to join our team. 

 
ESSENTIAL FUNCTIONS: 

1.      Provides case management, supportive interventions and community resources to patients, families and co-workers. 
2.      Maintains patient record in compliance with all governing bodies including DAODAS, DHEC, CARF and DHHS (Medicaid).
3.     Provides transportation for patients as needed in agency provided vehicle. 
4.      Meets with patients to determine eligibility status for Medicaid and assist in the application process if needed. 
5.      Responsible for writing letters and/or updates on patients, call patients and referrals for updates, assist with transfers and discharges, and assist patients with coordinating transportation. 
6.      Requires regular and reliable attendance. 
7.      Attend all mandatory training/licensure and receive an annual TB screening. 
8.      Proficiency with MS office computer software applications and/or Medical Records software applications, emails, etc. 
9.      Other duties as assigned by Supervisor. 
Compensation: $16.00 - $19.00 per hour
